Your first dental visit checklist
Three short lists: what to do before the day, what to bring, and what to tell us.
Before the day
- Book online or by phone, and mention your insurance.
- Look over the patient forms you’ll fill in.
- Ask your last dentist to send your records and any X-rays from the past year.
- Write down any pain, sensitivity or worries you want to raise.
- Under 18? Arrange for a parent or guardian to come with you.
What to bring
- A photo ID
- Your dental insurance card, or your plan details
- A list of your medicines and supplements, with doses
- Recent X-rays or dental records, if you have them
- Your reading glasses, for the forms
- A card, HSA or FSA card for any co-pay
What to tell us
- Any health conditions, even if they seem unrelated
- Allergies, especially to latex, penicillin or numbing
- If you’re pregnant or might be
- If you take blood thinners or bone-strengthening medicines
- If you feel nervous about dental visits
- Any problems with numbing in the past
Why your health details matter
A dental visit is safer when we know your health. These are the details that most often change how Dr. Ibrahim Sayeed plans your care.
| Tell us about | Why it matters |
|---|---|
| Blood thinners | They affect bleeding after an extraction or deep cleaning. |
| Heart conditions or a joint replacement | Some patients need antibiotics before certain treatments. |
| Diabetes | It raises the risk of gum disease and slows healing. |
| Bone medicines (bisphosphonates) | They change how the jaw heals after extractions and implants. |
| Pregnancy | X-rays and some medicines are timed or adjusted. |
| Allergies | We choose materials and medicines that are safe for you. |

On the day
- Arrive 10 to 15 minutes early to check in and finish your forms.
- Brush and floss as usual before you come, and eat normally unless we’ve told you otherwise.
- Your visit takes about 60 to 90 minutes, including your exam and any X-rays.
- You leave with a plan: what Dr. Sayeed found, your options and the costs in writing.

Checklist questions
Patients come to us from Kingston, Port Ewen, Lake Katrine, Hurley, Saugerties and across Ulster County.
Should I brush my teeth before a dental appointment?
Yes. Brush and floss as you normally would. It makes the visit more comfortable and doesn’t hide anything from the exam.
Can I eat before my appointment?
Yes, eat normally. If a treatment needs you to fast, we’ll tell you in advance.
How early should I arrive?
Arrive 10 to 15 minutes early for a first visit, so there’s time to check in and finish your forms.
How do I get my X-rays from my old dentist?
Ask their office to send your records and recent X-rays to us. With your written permission, we can request them for you.
What if I don’t have dental insurance?
You’re welcome. You’ll get the cost in writing before treatment, and you can pay by card, HSA or FSA, or spread it with CareCredit or Cherry.
